About
I am Dr. Mike Neal, an optometrist from Pennsylvania. For 20 years I operated an eye care practice that performed at a level people in the industry found hard to believe, and I did it the hard way: systems, hiring, and relentless attention to how every hour in the building was spent.
Along the way I built and sold companies, including a hiring company that helped hundreds of doctor practices find staff. I learned that what separates thriving practices from drowning ones is almost never clinical skill. It is operations: who does what, what gets automated, and what the owner stops touching.
Then AI arrived, and I fell in love. I did not take a course. I built. Today an agent I made runs my mornings, my meals, and my reminders from my text messages. Agents market my software businesses while I sleep. An agent rebuilt my infrastructure and cut the bill to nearly nothing. The efficiency I spent two decades squeezing out of a practice by hand is now available to any doctor for a fraction of the effort, if someone shows them what is real and what is noise.
That is the job now: helping practice owners get their time back. I work from a laptop, I answer straight, and I only recommend what I would run myself.
Off the clock
I am a dad of two boys, which is the real job.
In March of 2022 my longest run was couch to fridge. In October of 2024 I finished a 61-mile, 12-hour ultramarathon, and the following April I ran the Boston Marathon. I mention it not to impress you but because it is the same pattern as everything else on this site: when something matters to me, I go from zero to obsessive competence embarrassingly fast. Running got that treatment in 2022. AI got it next.
